About Us
The South West London (SWL) Interface Group was established in 2024 to strengthen collaboration between general practice and consultant-led teams.
Its purpose is to address high-priority issues at the interface between primary and secondary care, supporting more effective, coordinated, and patient-centred care across the healthcare system.
The Consensus Document below underpins our collaborative intent and shared set of principles.

The group is chaired by Dr Nicola Jones, Chief Medical Officer at South West London ICB and Clinical Director for Primary Care, South West London. It brings together senior representatives from across the SWL health and care system, including:

Explore Resources
The SWL Interface Group has developed, or is in the process of developing, a range of practical resources to support collaborative working between primary and secondary care, and improve patient care.

Looking Ahead
The SWL Interface Group will build on the progress already made by strengthening partnerships between primary and secondary care and supporting national and local interface priorities.
Through continued collaboration, the group aims to improve communication, streamline ways of working and enhance the experience of both patients and staff across South West London.
