Bridging the Gap Events
These events involve local GPs and consultants from each borough coming together to identify challenges, share insights and develop practical solutions that improve the patient experience and streamline clinical pathways and ways of working.
South West London Consensus Document
The Consensus Document sets out a shared ambition for how primary and secondary care can work together more effectively. It establishes a broadly agreed set of principles and aspirations that support collaboration and improve outcomes for both patients and staff.
Clinical Innovators Sharing Insights
Dr Arpa Patel, Dr Harriet Booth, Dr Matt Davis and Dr Shona Biggart are the SWL ICB GP Clinical Innovators. They work locally at the interface between acute trusts and primary care to identify and resolve local issues that sit between services - making pathways safe, more efficient and better for patients. They also contribute their expertise at national, regional and local level to help drive collaboration and shared learning.
