What are Primary Care Anchor Networks?

Primary Care Anchor Networks (PCANs) are a collaboration of primary care providers and organisations that work together to improve community health by addressing social determinants of health. These networks aim to create sustainable, community-centred health systems through shared resources, data, and strategies.​

Just as an anchor holds a ship in place, providing stability and direction, a PCAN anchors the health of a community. By connecting local primary care providers, it helps keep the community grounded in better care, guiding people toward healthier outcomes.
