This webinar will focus on detecting Heart Failure in primary care, with an emphasis on practical advice regarding diagnostic investigations and referrals. Dr Sue Piper, consultant cardiologist at King’s College Hospital, will discuss who and when to refer, the information to include in a referral, and treatment options that can be offered prior to diagnosis. The session will also explore current Heart Failure terminology and what primary care should expect after the Heart Failure review. All South London primary care clinicians are welcome and you are encouraged to bring your questions!

  • Assessment of the breathless patient (focus on cardiovascular causes but to include other recommended investigations).

  • Using NTproBNP in patients with suspected Heart Failure – pros and pitfalls.

Heart Failure nursing teams: what, where, and why?

The role of the Heart Failure Specialist Nurse (HFSN) has evolved significantly since a study carried out by Blue et al in 2001 of nurse-led post-discharge HF care demonstrated reduction in mortality, hospitalisation, and patient self-reported improved quality of life measures. HFSNs are highly skilled autonomous practitioners whose advanced skill set, knowledge, and clinical acumen support patients across several care settings. HFSNs utilise a holistic approach in the assessment and care provision of patients living with heart failure, supporting patients with their physical, psychological, social, occupational, and care giver needs from diagnosis to the end of life.

In short, HFSN are essential for delivering holistic, patient-centred care that improves both health outcomes and patient experience. This session will further explore the services offered by HF nurses, how to access these services, and how care is delivered.
