This masterclass has been designed to enhance participants' confidence and competence in the assessment and management of frailty within primary care and community settings. Drawing on contemporary frailty models and the principles of Comprehensive Geriatric Assessment (CGA), the session will provide a practical framework for identifying frailty and delivering person-centred care.
Participants will explore the key risks associated with frailty, including falls, delirium, medication-related harm and avoidable hospital admissions, and will learn how evidence-based risk stratification approaches can support early intervention and proactive management.
The masterclass will also focus on medicines optimisation in older adults, covering safe prescribing principles, recognition of high-risk medicines, and the use of structured medication reviews and deprescribing strategies where appropriate. Attendees will gain practical insight into managing common frailty syndromes, acute illness and complex multimorbidity, while supporting shared decision-making and individualised care planning.
